Connect Health + Wellness offers medical care to all children and adults regardless of income and insurance status. Medical care services include but are not limited to routine office visits for illness, annual physical exams, well-child checks, dental care referrals, immunizations & vaccinations, lab services, sports physicals and more. Additional services are behavioral health, medication assistance, case management, referrals, insurance assistance and health education. We welcome insured patients, including Medicare and Medicaid, and uninsured and those who have no insurance or are underinsured. Sliding fee scale based on household income is offered.
