At Evergreen Medical Wellness PLLC we help you reach your health goals so you can be your best self. We specialize in weight loss, hormone optimization for both men and women, and targeted wellness including peptides and hormone pellets. We address primary care type concerns as an appointment based immediate care clinic. You are welcome to book your appointment directly on our calendar for available appointment times, but we are also happy to see you over video for telehealth on other days/times as well. The clinic is in Smithfield, Utah for in-person visits, and telehealth is offered for the western United States.
