As a recognized Level 3 Patient-Centered Medical Home, we are putting our patients at the forefront of care and building better relationships between patients and their care teams, while reducing health care costs. The objective is to have a centralized setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. MEHOP Family Medicine also offers extended hours after 5pm Monday - Friday and is open on weekends. We accept Medicare, Medicaid and Private/Employer Based Insurance Plans. Uninsured fee for service discounts are available based on household income to assist with the cost of medical services if eligible.
