Community Health Connection is committed to meeting the healthcare needs of the northeast Oklahoma community. We provide the highest quality, affordable and accessible primary healthcare services in a culturally respectful and compassionate manner. We have a team of caring professionals that provide a welcoming environment for teens and young adults to receive primary health care, sexual and reproductive healthcare services. We accept private insurance, Medicaid and Medicare. We also serve individuals who are uninsured by offering a sliding fee scale to those who meet certain requirements.
