**REFERRAL REQUIRED** We provide a broad scope of care including managing chronic illnesses such as diabetes, heart disease, and heart failure. We provide preventative services including disease screening, cancer prevention, and immunizations. We provide urgent care for cough, stomach aches, musculoskeletal pain, etc. We offer specialized services including Medication for Opioid Use Disorder, Hepatis C treatment, and Formerly Incarcerated Transitions Program. We provide one on one visits and group visits. We offer our services in person and virtually. Our care team includes a range of highly trained professionals including peer support specialists, a social worker, family physicians, a physician associate, a nurse and a medical assistant.
