Upstate Family Health Center provides primary care services to the community with a focus on quality, integrity and person-centered care. We provide patients a haven to address all of their needs, including those that are traditionally non-medical. We utilize a collaborative approach to patient care that works to support all domains of wellness including physical, emotional, occupational, social, and environmental. This allows us to better serve, treat, and provide care as we strive to build a healthy community through empowerment and partnership with our patients. As a Federally Qualified Health Center (FQHC), Upstate Family Health Center accepts all Medicare and Medicaid insurance plans, as well as most major insurances.
