Walk ins Welcome. #1 Physician's Fee: $295.00 consisting of a) Initial Visit b)Venipuncture; Blood will drawn at the premises for six tests and the invoice will be sent to the patient by the Laboratory after three weeks. c) After three working days at the follow up visit, the form USCIS i-693 will be ready and given to the patient in a sealed envelope along with a copy of that document. #2 Laboratory Testing Fee consists of a) Measles/Mumps/Rubella Panel $15.00 , b) RPR $8.60 c) Quanti FERON TB Gold $55.00 d) Varicella IgG $15.20. e) Neisseria gonorrhoeae $33.00 f) Hepatitis B Surface Antigen $15.20 The Total Fee of $142.00 is payable 3 weeks upon receipt of the invoice.
