Thriving Germantown Community HUB is a multi-generational, multi-sector care coordination program connecting families with services and resources that benefit them in school, at home, and in their communities. Our experienced, compassionate family service coordinators provide support to the children and families in the DES community, helping families secure needed resources and navigate the complexities of the community, health, and social service systems.
