To make an appointment, please call 574-285-4585. Appointments are easy to make and they do need to be scheduled. The Jayco Family Care Clinic has more space and appointments than a normal health care provider. You should expect zero to limited wait time to be seen for your appointment. The Jayco Heath Center is an 8,000 Sq. Ft. facility that consists of a 2,600 Sq. Ft. fully-equipped gym, an occupational medical center with an independent entrance, waiting area, exam areas and a laboratory. The Family Health Clinic, in partnership with Goshen Physicians, consists of an independent entry, waiting area, three exam rooms, procedure room, restroom, lab area and provider stations.
