About
Located inside the Foster Medical Pavilion Pharmacy. What services are provided? Extensive education, patient interviewing, and counseling by a trained clinical pharmacist. This includes drug-food and drug-drug interaction monitoring. INR results are obtained and interpreted through a point of care INR device in clinic. Based on the INR result and patient interview, the pharmacist will make changes to the patient’s anticoagulation therapy as indicated. Initial visits (40 minutes) with follow up visits (20 minutes). Pharmacist prescribes warfarin, enoxaparin, or fondaparinux. Additional services include bridging recommendations prior to procedures.
