1st Care Management brings the clinicians directly to you! For those with limited access to a doctor’s office or for patients transitioning from the hospital back to their homes, we will bring a clinician to them. Our practitioners see our patients on the go, while working in very close coordination with our supervisory staff. To assist our patients even further, we collaborate with various home health agencies, medical equipment providers and other specialty providers.
