Community Health Centers (CHCFL) is a non-profit organization of Central Florida family health centers that provide high-quality, affordable medical, pediatric, dental and pharmacy care. Community Health Centers Four Corners is located in Cagan Crossings (next to Walmart) in south Lake County. This modern center provides comprehensive medical, dental, and specialty care for children and adults. We offer Family Medicine, Pediatrics, Adult Emergency Dental, Pediatric Dental, Behavioral Health, Optometry, Women's Care, Lab, and Pharmacy. In addition to preventive care, we also offer an on-site low-cost Pharmacy, exclusive to our patients.
