The Cornell Scott-Hill Health Center is proud to be a Patient-Centered Medical Home (PCMH). A PCMH is a system of care that creates a strong partnership between you and your personalized care team. Your team will be led by your primary care provider and may include nurses, health educators, specialists or any other medical personnel who can help you achieve your health and wellness goals.
