Rocky Mountain PACE (Program of All-Inclusive Care for the Elderly) is a managed health care plan exclusively for older adults. RM PACE participants receive primary medical care, medication management, physical therapy, specialty services, in-home care, social work assistance, and transportation to and from the RM PACE Day Center and medical appointments.
