We offer affordable medical, dental, and mental healthcare alongside pharmacy services as an integrated community health center. We also have locations in Basalt, Rifle, and Glenwood Springs, Colorado. Our patients can access family medicine, pediatric services, prenatal care, postpartum care, mental and behavioral healthcare services, dental healthcare, pharmacy services, and telemedicine. We accept Medicaid, private insurance, CHP+, and provide sliding scale payment plans. We can also help you enroll in health insurance. It is our mission to improve the health of our community by providing equitable and affordable medical, behavioral, and dental healthcare for all.
