The Tri-State Community Healthcare Center (Federally Qualified Community Health Center) aims to provide the highest quality healthcare to improve individual and community health. Our goal is to make access to high-quality healthcare practical and affordable for everyone. To ensure effective communication regarding health issues and care plans, we allow ample time for individuals, families, and care teams to communicate fully. We also coordinate integrated healthcare with individuals and all care teams and assure accessible hours for working individuals and families. Lastly, we support the well-being and needs of our staff and their families. We welcome patients with insurance, as well as those who are underinsured or uninsured.
