Medical care for homebound seniors. The goal is to achieve a quality of life at the current level of functioning. All new patients are assessed for eligibility or care preference. The support team can assist in establishing a visit. We offer wound care, diabetes, dementia, chronic care management and palliative care. Our wound care certified staff will ensure that wounds heal quickly. Providing care in the patients home environment ensures that all scheduled appointments are kept. We also arrange for laboratory testing, immunizations, telehealth visits, and physicals as needed.
